How is my information kept safe on the referral form?

When you fill out the SPoA form, you will be asked for some personal information, such as your contact information and details of why you are asking for support. To keep your information protected, we use an online platform called Typeform for the SPoA form. This platform is housed on a secure server and access is strictly controlled.

We make sure that your information is only seen by staff members who are responsible for putting you in touch with the right support. We store the information securely on Typeform, and referrals are passed onto the relevant service, then the information is permanently deleted from Typeform after 6 months.

As per Typeform’s Privacy Policy, ‘The Creator’ (YMCA DownsLink Group and West Sussex SPoA) is responsible for any personal data that is submitted. Typeform only processes this data on the creator’s behalf.

What happens after e-wellbeing send my referral form to the SPoA team?

SPoA takes confidentiality very seriously and will only share your information if it is needed to get you the right support.

So that an informed decision can be made about the best place for your support, your information will be shared with YES Service, YMCA Dialogue, and SPFT CAMHS. If you are a young person and we are worried about your safety, we may also share your information with:

  • Your doctor/GP
  • Your parent/carer
  • Your school
  • Children's Social Care (West Sussex Integrated Front Door)

If you are under 13, you must have your parent or carer's consent to fill out the referral form. This is explained in the form.
